THE FIRST STEP - CONTACT MY AGED CARE
Here is How to Apply for Home Care Package:
Go to My Aged Care myagedcare.gov.au/eligibility-checker
or phone them on 1800 200 422 to request an assessment.
You can call our office if you have any questions before you make the call.
We are always happy to give advice as we know how overwhelming it can be.
STEP 2 - YOUR ASSESSMENT WITH MY AGED CARE
My Aged Care will contact you with a time and date for your assessment.
This will decide if you are eligible for a Home Care Package and which package level best supports your needs.
There are 4 tiers.

Level 2 Home Care Package: Low-level assistance tailored to individual requirements.
Level 3 Home Care Package: Intermediate support for evolving care needs.

STEP 3 - THE OUTCOME OF YOUR ASSESSMENT, HOPEFULLY AN APPROVAL
Next you will receive a letter from My Aged Care telling you the outcome of your assessment. If approved you will be placed on a waitlist until your package becomes available. (Times vary)
STEP 4 - FIND A HOME CARE PROVIDER
When your Home Care Package is assigned, you can choose a Home Care Provider
(hopefully Bannister In Home Care) to deliver your services.
Or of course another Provider that is suitable for what you require.
Your Home Care Provider will work with you to develop a care plan and initiate services
to meet your needs and goals.
